<p>(HealthDay News) &#8212; Obesity is a major risk factor for left atrial enlargement, which increases the risk of atrial fibrillation, stroke and death, a new study shows.</p>
<p>Atrial fibrillation is the most common type of arrhythmia (an irregular heart rhythm or heartbeat).</p>
<p>Researchers analyzed data on 1,212 men and women, aged 25 to 74, in Germany who were followed for 10 years. The study authors concluded that obesity and hypertension cause structural and functional changes in the heart and are independent predictors of left atrial enlargement (LAE).</p>
<p>The highest incidence of LAE after 10 years was seen in obese people &#8212; 31.6 percent compared to baseline prevalence of 10 percent among all study participants.</p>
<p>The findings, published in the Nov. 17 issue of the <em>Journal of the American College of Cardiology</em>, confirm the strong association between obesity and LAE reported in previous research. Some of those studies found that excess weight may affect left atrial size at an early age, potentially predisposing young obese people to future heart problems.</p>
<p>The authors of the new study said early assessment and intervention, especially among younger obese patients, is crucial to prevent the premature onset of cardiac remodeling &#8212; changes in heart size, shape and function &#8212; caused by LAE.</p>
<p>But they noted that it isn&#8217;t clear how much weight management or moderate weight loss can improve LAE. Further research is needed.</p>

<p>Obesity and diabetes &#8220;are basically the two conditions of greatest concern for U.S. adults right now,&#8221; said study lead author Edward Gregg, chief of epidemiology and statistics in the CDC&#8217;s division of diabetes translation.</p>
<p>&#8220;They have been increasing for the last 10 to 15 years,&#8221; he said. &#8220;This is the first time we&#8217;ve been able to look at the most local level, defining where the problem is.&#8221; The findings are published in the Nov. 20 issue of the agency&#8217;s journal, <em>Morbidity and Mortality Weekly Report</em>.</p>
<p>In the Appalachia region that includes Kentucky, Tennessee and West Virginia, 81 percent of counties have high rates of diabetes and obesity. In the Southeast region that includes Alabama, Georgia, Louisiana, Mississippi and South Carolina, three-quarters of the counties have similarly high rates of both.</p>
<p>In many counties within those regions, more than 10 percent of residents have a diabetes diagnosis, and obesity prevalence is more than 30 percent.</p>
<p>Poor diets and sedentary lifestyles, both commonplace in modern American life, are known to contribute to type 2 diabetes and obesity.</p>
<p>&#8220;We think these increases in obesity and diabetes partially reflect the cultural shifts that are affecting all of us,&#8221; said Gregg, &#8220;but we see them most in regions where there is more poverty and where educational levels are lower.&#8221;</p>
<p>The two conditions go hand in hand, Gregg said. &#8220;Diabetes is one of the most tangible damaging effects of obesity. People who are very obese have four to five times the risk of developing diabetes than people of normal weight,&#8221; he said. Obesity is also linked to heart disease, stroke and some cancers.</p>
<p>The new CDC report coincides with data released Thursday by the Agency for Healthcare Research and Quality (AHRQ) that finds that two out of three people with diabetes forego either some or all of the most important examinations they need to stay healthy.</p>
<p>Using 2007 data, the agency found that only one-third of adult diabetics had had a health professional check their blood sugar levels that year, had undergone an eye exam, or had their feet examined for poor circulation. Only 58 percent had even two of these three crucial exams, the AHRQ found, 3 percent had had none of the tests, and 6 percent couldn&#8217;t tell researchers if they had had the tests of not.</p>
<p>Insurance status played a big role, with insured individuals being twice as likely to say they had had all three tests versus the uninsured.</p>
<p>Americans&#8217; expanding waistlines are taking a financial toll, too. The medical costs of obesity in the United States reached $147 billion in 2008, the CDC estimates, and people diagnosed with diabetes have medical costs that are more than double those of people without the disease. The CDC estimates that 8 percent of U.S. adults have diabetes, and another 5.7 million have it but don&#8217;t know it.</p>
<p>The new county-by-county information is a useful tool, Gregg said. &#8220;If people see the information that is closest to them, if we make it local to the person with the condition, the doctor, the legislator, that is better than seeing it at an abstract level,&#8221; he said.</p>
<p>&#8220;Awareness of the size and scope of the problem is important for local policy makers to identify the necessary community and clinical services to prevent and control the conditions,&#8221; the <em>MMWR</em> report said. &#8220;For example, lifestyle programs for diabetes prevention and community support groups for diabetes self-management have been shown effective when they are linked to a referring clinical center.&#8221;</p>
<p>The CDC said it has recommended 24 community strategies to prevent obesity and type 2 diabetes, the more common form of the disease that generally develops later in life as people lose their ability to metabolize sugar. These strategies range from providing greater access to healthy foods to promoting community redesigns that encourage more physical activity.</p>
<p>In Mississippi, which leads the nation in obesity and has one of the highest rates of diabetes, the agency is now working in partnership with state, federal and nonprofit agencies.</p>

			<content:encoded><![CDATA[<div>ALHAMBRA, Calif. (AP) — The fight against fat is going high-tech. To get an inside look at eating and exercise habits, scientists are developing wearable wireless sensors to monitor overweight and obese people as they go about their daily lives.</div>
<p>The experimental devices are designed to keep track of how many minutes they work out, how much food they consume and even whether they are at a fast-food joint when they should be in the park. The goal is to cut down on self-reported answers that often cover up what&#8217;s really happening.</p>
<p>In a lab in this Los Angeles suburb, two overweight teenagers help test the devices by taking turns sitting, standing, lying down, running on a treadmill and playing Wii. As music thumps in the background, wireless sensors on their chests record their heart rates, stress levels and amount of physical activity. The information is sent to a cellphone.</p>
<p>&#8220;I can&#8217;t feel my legs,&#8221; 15-year-old Amorette Castillo groans after her second treadmill run.</p>
<p>Traditional weight-loss interventions rely mainly on people&#8217;s memory of what they ate for dinner and how many minutes they worked out. But researchers have long known that method can be unreliable since people often forget details or lie.</p>
<p>The new devices are being designed in labs or created with off-the-shelf parts. Some similar instruments are already on the market, including a model that tracks calories burned by measuring motion, sweat and heat with armbands.</p>
<p>But the devices in development aim to be more sophisticated by featuring more precise electronics and sometimes even video cameras. Many emerging systems also strive to provide instant feedback and personalized treatment for wearers.</p>
<p>At the <a target="_blank" title="More news, photos about University of Southern California" href="http://content.usatoday.com/topics/topic/Organizations/Schools/University+of+Southern+California">University of Southern California</a> lab, the teens alternated between being sedentary and active as researchers resolved the technical bugs. Later this year, some will wear the body sensors at home on weekends. If they get too lazy, they will get pinged with a text message.</p>
<p>&#8220;We&#8217;ll be able to know real-time if they&#8217;re inactive, if they&#8217;re active,&#8221; said Donna Spruijt-Metz, a USC child obesity expert in charge of the project.</p>
<p>The devices are made possible by advances in technology such as accelerometers that can measure the duration and intensity of a workout. They also use Bluetooth-enabled cellphones that can take pictures of meals and send information back.</p>
<p>Will all this wizardry lead to a slimmer society? Scientists say there&#8217;s reason to hope. Getting an accurate picture of what people eat and how often they move around will help researchers develop personalized weight-loss advice.</p>
<p>Obesity is epidemic in the United States with two-thirds of adults either overweight or obese. It&#8217;s a major health concern for children and adolescents, who are at higher risk for high blood pressure, high cholesterol and diabetes as they grow older.</p>
<p>A federally funded pilot project by the Pennington Biomedical Research Center in Louisiana is exploring whether people can lose more weight when tracked by technology.</p>
<p>Participants carry around Blackberry Curves to snap pictures of their meals and leftovers. They also wear a quarter-sized device on their shoe that counts the number of steps they take.</p>
<p>Counselors pore over the incoming data and give individually tailored health advice through e-mail or telephone. Every month, the participants get their weight checked, and their progress is compared against a separate group that receives only generic health tips.</p>
<p>The study involves just seven people, but researchers eventually hope to have 40.</p>
<p>&#8220;It&#8217;s highly personalized. You get feedback very quickly,&#8221; said Corby Martin, who heads Pennington&#8217;s Ingestive Behavior Laboratory.</p>
<p>By using technology to capture eating and exercise details, researchers hope to bypass self-reporting that can sometimes give an incomplete picture.</p>
<p>But some medical experts are concerned about ethical questions. Even if people agree to be tracked, researchers worry about intruding into the rest of their lives and the lives of those around them.</p>
<p>&#8220;As a researcher, I&#8217;m a professional voyeur, and I like to find out whatever I can about human subjects,&#8221; said William McCarthy, a professor of public health and psychology at the <a target="_blank" title="More news, photos about University of California, Los Angeles" href="http://content.usatoday.com/topics/topic/Organizations/Schools/University+of+California,+Los+Angeles">University of California, Los Angeles</a>. &#8220;But if I were a subject, I&#8217;d be concerned about the level of detail that&#8217;s being captured about my behavior from moment to moment.&#8221;</p>
<p><a target="_blank" title="More news, photos about University of Pittsburgh" href="http://content.usatoday.com/topics/topic/Organizations/Schools/University+of+Pittsburgh">University of Pittsburgh</a> engineer Mingui Sun has developed a necklace equipped with a video camera that records where a person goes and what he or she eats. Before a researcher sees the data, it&#8217;s filtered by a computer that blurs out other people&#8217;s faces.</p>
<p>The device is not smart enough to know whether the wearer ate a Big Mac or tofu. So a researcher inputs the food, and the computer calculates the portion size, calories and nutrients.</p>
<p>Sun&#8217;s lab workers are wearing the prototype, and he hopes to test it on real people by the middle of the year.</p>
<p>Another concern is whether people, particularly youngsters, will stick with it.</p>
<p>Fellow Pittsburgh researcher Dana Rofey recently completed a study of 20 overweight female preteens and teens who wore armbands tracking the number of steps taken and calories burned daily.</p>
<p>Researchers found the armbands were worn 75% of the time. Though the study did not include a comparison group, researchers were pleased with the high compliance rate.</p>
<p>On a recent weekday, Castillo and another study volunteer, 13-year-old Eric Carles, headed straight from school to the USC lab, where they strapped the sensors on and went through a sort of circuit training. The project manager timed them as a postdoctoral student recorded the session through a one-way mirror.</p>
<p>Through periods of sitting, standing and exercising, they chatted about scary movies and upcoming exams. Wearing the devices felt &#8220;weird&#8221; to Castillo initially, but she has since grown used to it.</p>
<p>Castillo admits she doesn&#8217;t exercise as she often as she would like and has a sweet tooth for chocolate. Carles, who plays after-school sports, confesses he eats a lot. The teens were willing to try anything to help them lose weight.</p>
<p>After enduring more than two hours of required physical activity, the two were allowed to do whatever they want. Researchers called it &#8220;free living,&#8221; and it offered a glimpse into the activities teens would choose when they test the sensors at home.</p>
<p>The two chose to play a music video game. <p><strong>1. How active are you on a typical day?</strong></p>
<p><strong>A.</strong> Low-key. Unless you count walking to the car, mailbox and fridge or around the store or briefly at work, I&#8217;m either sitting, standing still or snoozing.</p>
<p><strong>B</strong> In gear. I&#8217;m often walking around at work, dashing up and down the stairs or chasing kids.</p>
<p><strong>C.</strong> In high gear. I move as much as I can throughout the day at work and at home.</p>
<p><strong>2. How much conscious exercise do you get in a week?</strong></p>
<p><strong>A.</strong> Not much. I rarely get to the point of breathing hard, let alone break a real sweat.</p>
<p><strong>B.</strong> I&#8217;m trying. At least three days a week, I can feel my heart rate going up when I walk or exercise.</p>
<p><strong>C.</strong> I&#8217;m trying hard. At least four days a week I work out for 45 minutes to an hour and often work up a sweat.</p>
<p><strong>3. How fit are you right now? </strong></p>
<p>If I warm up and then walk 1 mile at a hard but not painful pace, it takes:</p>
<p><strong>A.</strong> More than 19 minutes.</p>
<p><strong>B.</strong> 14 to 19 minutes.</p>
<p><strong>C.</strong> Less than 14 minutes.</p>
<p><strong>4. Which of the following best describes the outcome you&#8217;d like as a result of being more active?</strong></p>
<p><strong>A.</strong> I&#8217;d be happy to just be a bit healthier, to wake up less stiff each morning and to have a lower risk for chronic disease.</p>
<p><strong>B.</strong> I&#8217;d invest enough effort to receive all the benefits in (A), and also to begin losing weight at a noticeable rate — say, a pound every week or so.</p>
<p><strong>C.</strong> I&#8217;d work hard enough to get the health and weight-loss benefits of (A) and (B), and also to begin noticeably improving my aerobic fitness and strength.</p>
<p><strong>Calculate your Score </strong></p>

<p>Add all of your points up then figure out which level is best for you.</p>

<p><strong>If you scored &#8230;</strong></p>
<p>•<strong>4-6 points. </strong><strong>Beginner. </strong>Start at         <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#level1">Level 1</a>, gradually working up to a 20-minute walk. Keep a log of how long you walk and about how many miles you cover or steps you take. Next week add a few more minutes daily to work up to <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#leveltwo">Level 2</a>,         <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#levelthree">Level 3</a> and so on.</p>
<p>•<strong>7-9 points. </strong><strong>Intermediate.</strong> You may be ready to start at         <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#levelthree">Level 3</a> or         <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#levelfour">Level 4</a> with at least 20 minutes of walking a day. If this is too hard, cut back and work up to         <a target="_blank" href="http://www.usatoday.com/news/health/weightloss/2010-01-05-exercise-walking_N.htm?csp=usat.me#levelthree">Level 3</a>. If it&#8217;s too easy, then pick a higher level. Work on increasing duration, then speed.</p>
<p>•<strong>10-12 points. </strong><strong>Advanced. </strong> You may be able to do a 45-minute to 60-minute walk daily. If this feels like too much, start more gradually. If it&#8217;s too easy, pick up your speed or add other activities.</p>

<p>(HealthDay News) &#8212; Insomnia is a broad term that may include difficulty sleeping through the night, or problems falling asleep in the first place.</p>
<p>The National Women&#8217;s Health Information Center offers this list of possible treatment options for insomnia:</p>
<ul>
<li>Practicing      relaxation therapy to help manage stress.</li>
<li>Working      with a doctor, initially restricting the amount of sleep (rising earlier      or going to sleep later), then gradually increasing sleep to encourage a      full night&#8217;s rest.</li>
<li>Reconditioning      yourself for sleep, which involves limiting your bed for sleep or sex &#8212;      no TV or reading. This helps condition your body that your bed is only for      sleep.</li>
<li>Getting      a physical exam to see if there are any health conditions that could be      contributing to insomnia.</li>
<li>Evaluating      your lifestyle for behaviors that could negatively affect sleep, such as      drinking alcohol or caffeine at night.</li>

(HealthDay News) &#8212; Medical care for osteoarthritis patients in the United States costs $185.5 billion a year, according to a new study.
Of that amount, insurers pay $149.4 billion while patients pay $36.1 billion in out-of-pocket costs. Annual insurer costs are $4,833 per female patient and $4,036 per male patient. 			<content:encoded><![CDATA[<h3><strong>Improved screening, awareness may help delay progression and reduce expenses, researchers say</strong></h3>
<p>(HealthDay News) &#8212; Medical care for osteoarthritis patients in the United States costs $185.5 billion a year, according to a new study.</p>
<p>Of that amount, insurers pay $149.4 billion while patients pay $36.1 billion in out-of-pocket costs. Annual insurer costs are $4,833 per female patient and $4,036 per male patient. Women also have higher out-of-pocket expenses than men &#8212; $1,379 versus $694. The total cost for female patients is $118 billion, compared with $67.5 billion for male patients. All figures are in 2007 dollars.</p>
<p>&#8220;Understanding the economic costs of OA [osteoarthritis] is important for payers, providers, patients and other stakeholders. Our study clearly reflects the significant impact of OA on U.S. health-care spending,&#8221; study author John Rizzo, of Stony Brook University in New York, said in a news release.</p>
<p>For the study, which is published in the December issue of the journal <em>Arthritis &amp; Rheumatism</em>, Rizzo and his colleagues analyzed 1996-2005 data from the Medical Expenditure Panel Survey. The data sample included 84,647 women and 70,590 men aged 18 and older who had health insurance. The health-care costs included physician, hospital and outpatient services, as well as drugs, diagnostic tests and related medical services.</p>
<p>People with osteoarthritis suffer gradual loss of cartilage, primarily in the knees, hips, hands, feet and spine. About 27 million Americans have osteoarthritis, which affects more women than men, according to the U.S. Centers for Disease Control and Prevention. By 2030, it is projected that 25 percent of the U.S. population (nearly 67 million people) will have physician-diagnosed arthritis.</p>
<p>The study authors said increased awareness and better screening to identify patients with osteoarthritis may help delay disease progression and resulting disability, thus reducing medical costs.</p>
<p>&#8220;Our results suggest that patients with OA may benefit from greater efforts to promote exercise, proper medication use and appropriate surgical treatments for the disease,&#8221; Rizzo concluded.</p>

<p><strong>Dermatologists discover gender differences that explain the aging inequity</strong></p>

<p>(HealthDay News) &#8212; Dermatologists have discovered yet another gender inequity: Women develop more and deeper wrinkles around their mouths as they age than men do.</p>
<p>The disadvantage had long been suspected, but a new study provides &#8220;irrefutable scientific evidence,&#8221; said Dr. Foad Nahai, a plastic surgeon practicing in Atlanta and editor-in-chief of the <em>Aesthetic Surgery Journal</em>. The study appears in the November/December issue of the journal.</p>
<p>The authors of the paper believe they even know why women suffer more in this department.</p>
<p>&#8220;The gender differences were most probably due to the amount of appendages [hair follicles, sebaceous glands] and the connections between the skin and muscle of the lips,&#8221; explained senior study author Dr. Moshe Kon, head of the department of plastic, reconstructive and hand surgery at University Medical Center Utrecht in the Netherlands.</p>
<p>&#8220;We had always had the impression that male skin doesn&#8217;t age as rapidly as female skin,&#8221; said Nahai. &#8220;What we didn&#8217;t know, and this study points out, is that one of the reasons that lines are deeper in women is because they have fewer sweat or sebaceous glands, the glands that make the oil that keeps our skin feeling soft &#8230; So women are producing less oil, which is protective and keeps the skin smoother.&#8221;</p>
<p>Previous studies had not focused in on differences between men and women in the perioral area of the face, meaning the region around the mouth.</p>
<p>Women tend to go for procedures to remedy such wrinkles more often than men, although it hadn&#8217;t been clear if women were just worried about how they looked or if their wrinkles were actually worse.</p>
<p>The authors studied the skin around the upper lip in both male and female cadavers, as well as reconstructions of those areas.</p>
<p>Several key differences between men and women emerged: women had fewer sweat glands around the mouth than men; women had fewer blood vessels so less blood flow to that region; the muscles around the mouth are closer to the skin than in men, which may pull the skin in tighter, causing wrinkles; and, although the number of hair follicles were about the same in both genders, men had more sweat glands per hair follicle, again contributing to more relaxed skin throughout aging.</p>
<p>Hormones also seem to play a role in women&#8217;s dermatologic aging process.</p>
<p>&#8220;A lot of gender differences in health and disease are related to estrogens,&#8221; said Dr. Seth Thaller, chief of plastic surgery at the University of Miami Miller School of Medicine.</p>
<p>For instance, estrogens promote healing, meaning that women tend to heal from wounds faster. And postmenopausal women have reduced blood flow, again contributing to lines and furrows. And they experience a decrease in the fat (sebum) secreted by sweat glands.</p>
<p>Meanwhile, women on hormone replacement therapy have been reported to have fewer wrinkles than those not taking the hormones.</p>
<p>Something the authors did not mention were changes in the bone, which also contribute to more severe aging, said Dr. Jessie Cheung, associate director of cosmetic dermatology at NYU Langone Medical Center in New York City.</p>
<p>&#8220;As you age, your jaw bone actually gets resorbed slowly so the bone shrinks down. That contributes a lot. The skin is going to get saggy because there&#8217;s no scaffolding holding it up like a tent,&#8221; she said.</p>
<p>Tissue also gets thinner as people age.</p>
<p>Many existing cosmetic procedures, including fat transfers, can remedy some of these changes, Cheung added.</p>
<p>But, said Nahai, &#8220;the best thing anyone can do to keep their skin young is avoid smoking and avoid the sun and keep it moist. Use moisturizers and use skin creams.&#8221;</p>
<p>&#8220;Our findings might stimulate industry to develop applications which take the differences between men and women into consideration [like the use of specific hormones],&#8221; added Kon.</p>
